What Is XERF?
XERF is a non-surgical dual-frequency radiofrequency treatment from Cynosure Lutronic. It delivers 6.78 MHz to the upper dermis and 2 MHz to deeper tissue, heating the skin to contract existing collagen and stimulate new collagen over the following weeks. Patients use it for jawline definition, lower-face and neck tightening, fine lines and skin firmness.
FDA clearance: XERF received 510(k) clearance in August 2025 (K251327) for use in dermatologic and general surgical procedures for electrocoagulation and hemostasis; tightening effects occur via soft tissue coagulation.
Collagen stimulation works best when your skin barrier is strong going in which is where preparation comes in.
Before XERF: Your Pre-Care Checklist
1–2 weeks before
- Strengthen your barrier. We often recommend SkinCeuticals Triple Lipid Restore 2:4:2 formulated with 2% ceramides, 4% cholesterol and 2% fatty acids that mirror the skin’s natural lipids to support barrier resilience and comfort during treatment. Start it 1–2 weeks ahead.
- Stay out of the sun and skip tanning. Treating recently tanned or sunburned skin raises the risk of irritation.
5–7 days before
- Pause retinoids and exfoliating acids (retinol, tretinoin, glycolic, salicylic). This is the most important pre-care step — treating over actively-exfoliating skin increases sensitivity and the risk of an adverse response.
2 weeks before
- Space out injectables. Schedule XERF at least two weeks after any neurotoxin or filler in the same area (and confirm timing with your provider).
24–48 hours before
- Skip alcohol for 24 hours to reduce swelling.
- Arrive with clean skin — no makeup, lotion, SPF or self-tanner in the treatment area on the day.
- Tell us about cold sores. If you’re prone and we’re treating near the mouth, ask about preventive antiviral medication.
We’ll also take baseline photos insist on this, because you won’t accurately remember your starting point.
On the Day: What to Expect
Your provider applies a conductive gel and glides the handpiece across the treatment area in passes, adjusting depth and intensity by zone. Most face-and-neck sessions run 30–45 minutes. You’ll feel a deep, moving warmth as the cooling cycles; most patients don’t need numbing. Tell your provider if anything becomes genuinely uncomfortable settings should be adjusted, not endured.
After XERF: Repair, Support, Protect
Your skin goes into repair mode immediately. Good aftercare supports recovery and protects your developing result.
The first 24–48 hours
- Expect mild redness or warmth — usually gone within hours, occasionally a day.
- Support recovery. We often recommend SkinCeuticals Advanced RGN-6, which contains peptides and growth-factor technology to support the skin’s natural repair and complement collagen remodeling.
- Be gentle. Bland, non-irritating cleanser and moisturizer only.
- Avoid heat — no saunas, hot yoga, steam rooms or very hot showers for 24–48 hours (heat can add to redness and, over melasma-prone skin, aggravate pigment).
- No makeup for the first several hours if your skin feels sensitive; otherwise makeup is generally fine same-day.
Days 2–5
- Reintroduce actives gradually after 2–3 days, once any sensitivity settles barrier support first, then work back to your routine.

- Hydration and protein support collagen synthesis from the inside.
Your Results Timeline
- Immediately: many patients notice subtle tightening (partly a temporary heat-contraction effect)
- 4–6 weeks: new collagen becomes visible; skin begins to feel firmer

Who Should Not Have XERF
XERF isn’t right for everyone. Tell your provider if any of these apply some exclude treatment, others change the plan:
Pregnancy or breastfeeding · pacemaker or other implanted electronic device (a specific radiofrequency concern) · metal implants or permanent filler in the treatment area · active infection or open wounds · history of keloid or hypertrophic scarring · autoimmune or connective tissue disease · uncontrolled diabetes or impaired wound healing · recent isotretinoin use · active or unstable melasma in the area.
